• US is not typically used to evaluate calcs found on mammography unless there is an associated mass or calcs are extensive (usuallys spanning >2 cm)  

• US can be useful in the setting of highly suspicious calcs to look for a mass that may represent invasive component and potentially guide bx

• Purpose

  • Screening - asx w/o personal h/o breast CA, women w/ dense breasts

  • Diagnostic - abnormal findings on screening studies, signs & sx of breast CA, h/o breast CA, implants

    • Evaluation of palpable abnormalities

    • Determine if lesion is real

    • Characterizing masses

    • Determining extent of dz (staging)

  • Limited role in evaluating calcs

• Staging Sono: Looks for satellite lesions, multifocal (>1 lesion in same quadrant), multicentric (>1 quadrant involved) dz

• US following MRI

  • Lesions detected on MRI may be mammographically occult

Echogenicity is relative to the SQ fat
